Free Marine Order 504 (MO 504) Passenger Manifest Template

What the manifest must include (cl 15(8))

For the vessel: its name and unique identifier (UVI), and the voyage. For each person on board: their name, address (local and home if they have both), email (if any) and phone number — plus any medical or safety needs that could matter in an emergency.

When you need one

A named passenger manifest is required for voyages of 12 hours or more (MO 504, Sch 1 cl 15(7)). For shorter voyages a passenger count may apply instead — but you must still be able to account for everyone on board at any time.
