What Marine Order 504 requires
Your risk assessment must be prepared in consultation with the master and crew, kept readily accessible and up to date, and reviewed if the operation changes, after a marine incident, or if the master considers the risks have changed (MO 504, Sch 1 cl 2). It must also identify your key daily tasks, unacceptable risks, fatigue, when lifejackets must be worn, and your crewing.
What's in the template
A vessel-details header, an "operational risk profile" capturing the cl 2(3) items (key daily tasks, fatigue, lifejackets, crewing), a full hazard register (hazard, consequence, likelihood, rating, controls, residual risk, responsible person, review date), and a risk matrix.
